GCS 14-15.
He complained every now and then that he can't breathe despite having a normal SaO2.
I basically did lots of hrly ABG as I feel like something was not quite right, his PO2 was not great on HFNP, and his Lac was trending up.
I keep escalating as it doesn't feel like the breathing is comprised by pain too much.
Eventually his Lac was up to 5 and everybody finally taking my escalation more seriously.
I know he is a bit confused but probably not that confused to mistaken his difficult breathing.
In the end we gave some extra fluid, change his lines in case there's sepsis involved and start some IVAB and he gradually getting better the next day.
He also has urgent Echo and CCR.
Also during the handover in the morning, he had a little asystole probably caused by pacing box manually paused. I pressed the bottom and get Ppl to grab the trolley, and we tried to turn the pacing up and the issue was resolved.
A very busy day for sure, glad he was ok now.
